Appointment and Referral Information
When your doctor schedules you for an office visit with one of our surgeons, we would like to provide information to help you prepare for the visit. We are respectful of your time and understand the importance of providing you value in both service and high quality surgical care. During the scheduling of your appointment, we will ask for your contact information including address, phone, emergency contact, employer, insurance, and referring and primary care physician information. This information is necessary to begin the registration process and it is important that you bring this information with you when you arrive for your scheduled visit.
A telephone call will be made to you, prior to your scheduled visit as a reminder. If your scheduled appointment needs to be changed, please notify our office and we will be happy to reschedule. The office number is 517-267-2460, M-F 8 am to 5 pm.
On the day of your appointment, we ask that you arrive 15-20 minutes prior to your scheduled time to complete the registration process. Parking is located in the ramp, adjacent to the Sparrow Professional Building, located across from Sparrow Hospital.
We do encourage you to use our free valet parking, located in a circular drive on the west side of the Sparrow Professional Building.
Once you park, please enter the main lobby. Wheelchairs are available for your convenience. You will be directed to the elevators that will take you to the 6th floor, Suite 655. Enter the patient waiting room and register with the receptionist at the front desk. Copies of your contact information will be made at this time and you will be asked to complete a form about your past and current health status.
If you choose, you may download the medical history form and complete it prior to arriving. Please bring the completed form with you to your appointment.

During this initial visit, you will encounter other members of our health care team, including professional nurses, medical assistants, receptionists and students. Resident doctors, working with your attending surgeon, will also participate in your care.
Insurance Information
Our Patient Accounts Department is responsible for physician fees and for billing your insurance carrier. If you have medical insurance, our staff will verify your information and identify any co-pays or balances owed. Insurance co-pays are collected at the time of visit. If you have problems paying for your bill, we will assist you in making arrangements for payment.
